- 1 MONTH
- 2 MONTH Pediarix, HIB, Prevnar, Rotavirus
- 4 MONTH Pediarix, HIB, Prevnar, Rotavirus
- 6 MONTH Hemoglobin, Pediarix, HIB, Prevnar, Rotavirus
- 9 MONTH
- 12 MONTH Hemoglobin, Varivax, Prevnar, Hep A
- 15 MONTH MMR, HIB
- 18 MONTH DTaP, Hep A
- 2 YEAR Hemoglobin
- 3 YEAR Hemoglobin
- 4 YEAR Hemoglobin, Urinalysis, Vision & Hearing screen, DTaP, IPV, MMR, Varivax
- 5 YEAR Hemoglobin, Urinalysis, Vision & Hearing screen (DTaP, IPV, MMR, Varivax if not given at age 4)
- 6 YEAR + Yearly checkups recommended
- 11 YEARS Hemoglobin, Urinalysis, Tdap, Menactra
